He is preceded in death by his parents, Donald and Betty Cook.

Michael is survived by his daughter, Alicia Hall and husband, Jesse; sons, Ethan Cook and Casey Cook; brothers, David Cook and Lonnie Cook and wife, Cindi; grandchildren, Parker Hall and Harper Hall; several nieces and nephews; and a very special friend, Scott Boyer.
